The eForms portal is the home of both electronic and PDF forms related to Registrar and enrollment-based requests.

To find and submit forms, you’ll need to navigate to Student eForms, located inside of your My Academics drop-down menu within Axess.


On the eForms landing page, click on Browse Available Forms to submit a petition or form.


What is the status of my submitted form?

You can track the progress of your form(s) using the circular diagrams displayed in a variety of locations across the eForms portal. The circles represent the office(s) waiting to approve the form (i.e. "waypoint"). Green circles indicate that the listed office has completed their review and approved the request. A blank circle indicates that the form is still pending approval from that office. Forms are routed in the listed order from left to right. A form will not be routed to the next waypoint until the prior waypoint has submitted their approval. Once all waypoints have approved, the form will be routed to the Registrar’s Office for final approval and processing.
